Location: Castle Street, Merthyr Tydfil, CF47 8AN
Contract: 9th April 2025 – 24 Weeks (Ending Approx. 24th September 2025)
Hours: 37 hours/week, Monday-Friday (Times TBC)

Provide needs-led support to young people, helping them transition from care with sensitivity and equality.
Act as a personal advisor, creating and reviewing pathway plans for those over 18, ensuring clear objectives.
Assess and support their health, education, and financial needs, including managing allowances and payments.
Offer direct support with independent living, education, training, and income maximisation.
Build positive relationships, coordinate family time, and promote life skills through individual and group sessions.What We’re Looking For:
Skills: Strong communication, advocacy, and assessment skills, with a focus on young people’s needs.
Qualities: Empathetic, flexible, and committed to supporting care leavers in a mobile role.
Experience: 2+ years of relevant work history with references.
Qualifications: Social Care Wales registration, valid DBS check, and eligibility to work in the UK.
Bonus Points: Experience with pathway plans, financial assessments, or supporting young people post-care.
