Join our dedicated team at Derbyshire County Council, with 2 exciting opportunities to join either the Bolsover Short Term Assessment and Reablement Team (START) or the Bolsover North Long Term Social Work Team.The Bolsover START is a fast-paced and highly specialist team who are focused on the wellbeing, empowerment and reablement of people in our community. Wherever possible we aim to prevent the need for hospital admissions and entry into longer term care settings and enable people that are stepping down following a hospital stay, helping them to achieve their full potential to live as safely, independently and as well as possible in their own homes. We take a multi-disciplinary approach, and the team is a mixture of Social Workers, Social Care Practitioners, Occupational Therapists, Enablement Leads and Enablement Support Workers, working closely alongside our health colleagues.The Bolsover Long Term Social Work Team work with a wide range of adults who have care and support needs, including those approaching adulthood, both in the community and in care placements. Our approach is rooted in person-centred assessments, with a focus on Strength Based Models of practice and principles outlined in the Care Act 2014.Key Responsibilities 🤝– Conduct person-centred assessments, working closely with individuals and their carers to gather information and positively influence wellbeing.
– Undertake personalised safeguarding enquiries, demonstrating strong communication and organizational skills.
– Collaborate with other organizations and professionals, fostering effective multi-disciplinary practice.
