OverviewAs a Social Care Assistant working within the Home Independence Team, you will be working out in the community to support residents that have been recently discharged from hospital or those who are living with a disability and need help to live independently and establish a routine to help them manage their lives day to day. You will be supporting individuals to regain confidence and reach their goals of living independently again.We are looking for a dedicated individuals who can provide high quality direct care and support to residents within, focusing on personal wellbeing outcomes and individual wishes, to achieve strength-based support.Positions Available:

About usThe Home Independence service supports individuals to regain their independence following illness, disability, or recent stay in hospital. We work with clients in their homes, identifying whether they now have difficulties with self-care or performing daily activities. We provide a goal-based assessment for each client and alongside them to support the achievement of their goals, using technology-enabled care devices and occupational therapy aids to enable them to achieve independence.The team works closely with healthcare professionals, carers, clients and their families to develop suitable packages of support, designed to enhance client independence and meet their needs. We supply advice on specialist equipment options, monitor client progress and make necessary adjustments if required. If you enjoy working in a fast-paced environment alongside individuals who care about the wellbeing of our residents, the Home Independence team may be for you.About the roleNo two days will be the same as you meet a variety of people and support their individual needs. About youNaturally, you will be a great people person, caring and empathetic as well as being able to deliver practical care and support to an agreed plan. You will use your own initiative to problem solve and utilise remote management oversight, where required.You may need to undertake a specialist role that requires specific training, such as Autism services or undertaking and administering medication.
