About the TeamThe Reablement Service is part of the Adults Wellbeing and Health Partnerships directorate. We are a multi-disciplinary team who provide short term support for up to 6 weeks to people, usually in their own homes, to promote independence and choice by helping them work towards their reablement goals as set out in their support plan. All roles work in a way that seeks to improve people’s lives by working on their strengths, rather than their weaknesses, and making use of the community and networks around them, as well as local authority-provided services. We work closely with some private providers across Surrey who are part of our Collaborative Reablement Service (CRS). CRS is an extension of our Reablement Teams – by having CRS partners we have more capacity to support more Surrey residents.About the RoleAs a Reablement Team Leader you will be responsible for coordinating the day-to-day work of the team, to deliver a high-quality service in accordance with regulatory requirements. This will include triaging all cases needing reablement support and managing a small team of Reablement Assistants and Integrated Reablement Workers to support people to achieve their support plan goals.
