I feel welcome and at home.”View more comments from our clients and their family members.Pay RangeOPSEU/VHA Pay Range: $24.19 – $28.61To provide program support, coordination and scheduling to the Extensive Needs Service Respite program and other programs in the portfolio during staff absences or high-volume situations. To perform all intake functions: receive, assess and process all requests for service from potential clients and referral sources, provide information regarding the criteria for service, the range of services offered and referrals to other community services, provide and participate in outreach activities. Participates in VHA services and quality initiatives. All activities are conducted within the policies, procedures and standards of VHA.Key Areas of Accountability:
- Provides support in planning and scheduling service for clients
- Receives inquires for services and conducts intake assessment
- Develops and maintains relationships with referral sources and community resources
- Provides support for administrative activities
Key skills, experience and behaviors required for this position:
- Knowledge of Community Social Services and Community Health Care (a diploma or degree) combined with a minimum of 3 years’ experience including coordination of service and intake assessment
- Knowledge of medical terminology and psychosocial assessment tools
- Knowledge of Autism Spectrum Disorder, Mental Health and Addictions
- Good problem solving skills, solution-focus approach and crisis intervention skills
- Ability to handle a high volume of telephone intake calls and inquiries.
- Knowledge of Microsoft Office and the use of scheduling programs.
- Good keyboarding skills and accurate data entry skills.
- Strong attention to detail and organizational skills.
- Good verbal, written and interpersonal communication skills.
- Ability to juggle multiple priorities.
- Knowledge of the GTA and TTC routes.
- Demonstrate efforts to continuously develop and upgrade skills.
- Demonstrated commitment to client and family centered care.
