As a progressive and vibrant healthcare organization, we are committed to being an innovative center of excellence in community hospital care.Job DescriptionProvide safe, timely and efficient pharmaceutical care, patient education, drug information and support services to the patient care team. Participate in direct patient care, patient education, and collaborative clinical decision making with care team.
- Review medication orders, patient profile, and treatment regimen within Meditech using Oncology module for clinical appropriateness, distribution effectiveness, and billing accuracy
- Ensure safe, timely and efficient processes for compounding, and dispensing of medication used for Cancer Clinic, Medical Day Care and inpatient needs
- Support distribution activities (e.g., staging, checking, etc.) together with pharmacy technicians as required
- Maintain accurate documentation of administrative related tasks (e.g., eClaims, etc.) for reporting (e.g., financial)
- Timely documentation of patient assessments, medication reconciliations, clinical interventions and pertinent therapeutic notes within the patient electronic record
- Support maintenance of adequate inventory levels, and minimizing drug wastage together with pharmacy technicians.
- Work collaboratively with other pharmacy staff and health care providers to develop institutional guidelines and make evidence-based decisions designed to improve patient care, enhance safety, and improve quality of care.
- Participate on committees (including at regional level) to develop policies and implement programs to ensure the safety of staff and patients during the receipt, preparation, administration, and monitoring of anticancer agents.
- Maintain up-to-date awareness of current CCO-related communications (e.g., treatment funding), latest evidence based research, and best practice recommendations
- Participate in routine system and process maintenance based upon relevant policy, workflow requirements, and best practice recommendations
- Support pharmacy departmental objectives and act as clinical subject matter expert for other pharmacy staff
- Support Drug Access Facilitator (DAF) clinical and administrative duties as required
- Provide clinical and drug distribution support to inpatient oncology unit as required
- Assist with troubleshooting of clinical, distributive, billing, and system issues as required
- Occasionally provides back-up pharmacy service coverage of inpatient units or departments
- Other duties as assigned.

- BSc. Phm or PharmD required
- Licensed under Part A of the Ontario College of Pharmacists register required
- Hospital pharmacy residency preferred
- Strong knowledge of oncology therapeutics and current best practice guidelines, including symptom management and supportive care
- Strong clinical assessment and medication counseling skills
- Recent experience providing drug preparation and Pharmacy clinical services in an Oncology outpatient care setting preferred
- Experience with electronic prescription billing and computerized provider order entry (CPOE) systems preferred
- Meditech computer experience an asset
- Experience in the development of formal written policies and procedures and educational materials preferred
- Dispensing skills; knowledge of aseptic technique including preparation and handling of hazardous medications and experience with the practical application of USP 797/800 and NAPRA standards
- Familiar with the provincial reimbursement model for cancer care funding and Cancer Care Ontario eClaims
- Demonstrated high degree of accuracy and attention to detail
- Excellent problem recognition and problem solving skills
- Experience with application of Lean methodology an asset
- Able to handle multiple responsibilities with competing priorities in a fast-paced progressive environment
- Must be well-organized, self-directed, set priorities, and manage time effectively
- Must be able to interact patiently, effectively, and professionally with staff and physicians
- Committed to service excellence and patient safety
- Excellent verbal and written communication skills
- Demonstrated ability to participate effectively in a multidisciplinary team environment
- Must be able to meet the physical demands of the position

Additional InformationHours: Monday to Sunday, various rotating 7.5 hour shifts (0800-1600 and 1000-1800. May occasionally require some off-shift or weekend work.
