You will be part of a diverse care team that will provide services to a roster of clients in the community. You be part of a collaborative team that will conduct joint care planning with a focus on client-centered goals addressing client’s physical, cognitive, social, emotional and spiritual domains, creating a holistic care plan. Each client is assigned a primary care clinician, a member of the interdisciplinary team who will lead care planning for that client and facilitate interdisciplinary team to reach client’s care goals. • Conducting assessments
• Formulating goals and recommendations
• Planning and implementing client-centered, goal-directed treatment programs

Requirements

• Recent experience in managing patients in acute/rehab or community-based rehabilitation programs is preferred
• Reliable, flexible, creative, self-directed professional, able to work effectively and collaboratively with rehab assistants as well as with a high-performance interdisciplinary rehabilitation team
• Sound knowledge of community resources is an asset
• Effective interpersonal, organizational and communication skills
• A positive work and attendance record is essential
• Ability to adapt to new computer applications
• This role follows a hybrid work approach where you are able to work remotely and with the clients in their acute/rehab or community-based rehabilitation setting
• A vehicle and valid driver’s license and the ability to travel
