Mental Health Case WorkerThe Mental Health Case Worker is responsible for providing direct support to clients by using a harm reduction, client-centered, anti-oppressive, and trauma-informed approach. This position focuses on helping clients achieve housing stability, enhance their well-being, and build the skills necessary to live independently in the community.Key Areas of Accountability:
- Provide long-term clinical case coordination services to complex population of clients experiencing mental health, addictions, disabilities, and/or hoarding behaviors in collaboration with the client’s care team
- Conduct assessment of home support needs amongst program participants and collaborate with modified cleaners to support the process and execute the specific care plan need
- Implement and execute service plans steps created by or in collaboration with Supervisor and clients
- Participates in the investigation of incidents and work-related illnesses
Key skills, experience and behaviors required for this position:
- Degree or diploma in a health-related field, such as Social Work/Social Service, Psychology, Mental Health and Addictions Counselling or the equivalent combination of education and experience working with mental health vulnerable populations
- At least two year’s clinical experience working in the community and providing case coordination services to vulnerable clients that have been homeless, living in poverty, with mental health, substance use, hoarding behaviors, trauma, social isolation, unemployment/underemployment
- Utilizes an evidence base to clinical practice and possesses excellent consultation and teaching skills
- Familiar with the routine use of valid and reliable outcome measures to support clinical interventions
- Strong knowledge of community social and health services and established contacts with other agencies
- Strong computer skills including Microsoft Office and ability to learn VHA’s client management software system
- Proven ability to communicate effectively and build relationships with stakeholders at all levels
- Strong sense of accountability and ownership of program deliverables
- Ability to multi-task, stay on track with expected deliverables, and problem-solve in a fast-paced environment
- Highly motivated self-starter who takes initiative, is results-oriented and able to prioritize and work well both independently and as part of a team
- Excellent interpersonal and communication skills
- Proven ability to work from a client centered and harm reduction approach and has excellent problem-solving, conflict resolution skills, crisis intervention techniques and uses a solution-focused approach
- Must have a valid Ontario driver’s license, access to a vehicle and willingness to travel in the Region of Peel
- Flexibility to provide services outside of normal business hours based on client needs
