Key Responsibilities:

• Administer medications & treatments as prescribed, track vital signs, and document care
• Provide daily personal care & support per Individual Service Plans (ISP)
• Coordinate medical appointments & follow-ups, ensuring top-quality care
• Order & manage medications, supplies, and equipment to meet healthcare needs
• Respond to medical emergencies using CPR, first aid, and other life-saving techniques
• Train direct care staff on medical equipment use and treatment procedures

Requirements:

• Active LPN License in MA
• At least 3 months of direct care experience (healthcare, group home, or residential setting)
• Basic Life Support (BLS) Certification (or willingness to obtain)
