A-Line Staffing is now hiring a Field Case Management Coordinator in Princeton, IL (Bureau County).

• The pay for this position is $40.86 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility datesCase Management Coordinator Highlights
• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required availability for this position is Monday through Friday, 8:00 AM – 5:00 PM
• Field-based role with required travel within Bureau County; work from home between member visitsCase Management Coordinator Responsibilities
• Conduct comprehensive evaluations of referred members using care management tools, data review, and holistic assessments
• Identify high-risk factors and recommend interventions or referrals to internal/external services
• Coordinate and implement care plan activities, monitor progress, and document care per company and regulatory standards
• Use motivational interviewing techniques to promote healthy lifestyle choices and member engagement
• Collaborate with multidisciplinary teams including case managers, supervisors, and medical directors to overcome care barriers
• Provide education and support to help members actively participate in healthcare decision-making
• Identify and escalate quality of care issues when needed
• Utilize negotiation and critical thinking skills to ensure optimal and cost-effective member outcomesCase Management Coordinator Requirements
• Bachelor’s degree in behavioral health or human services (psychology, social work, marriage and family therapy, nursing, counseling, etc.)
?— OR —
• Non-licensed master’s-level clinician in a related field
• High School Diploma or GED (required for verification purposes)
• Must live near Bureau County due to travel requirement
• Must be organized, efficient, and able to work independently
• Attendance is mandatory for the first 90 days

Preferred Qualifications
• Experience with case management and managed care
• At least 2 years of experience in behavioral health, social services, or a related field
