We are a dynamic and fast-paced team comprising of Social Workers and Social Care Assistants working with multiple professionals to ensure safe discharge and follow up care. About the RoleAs a Senior Social Care Assistant you will have frequent direct contact with the public, visiting our residents and their families on the ward, speaking with them on the phone and in some cases visiting our residents in their own home or care setting. Your role will involve a variety of tasks including assessing and reviewing people’s needs to promote and expediate safe hospital discharges, in collaboration with our Health colleagues.You will work in partnership with individuals, their carers, professional staff in Surrey County Council and other agencies and service providers, in order to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control.In this position, you will mainly be supporting bank shifts that arise on weekends and bank-holidays. These shifts are based on site and you will be working alongside one or two other Adult Social Care Practitioners to support discharge flow through the Acute.Shortlisting CriteriaTo be considered for shortlisting for this position, your application will clearly evidence the following:

  • Substantial experience of working directly with people with social care needs, including completing strength-based and person-centred assessments and visiting them in their own homes or care settings
  • Confidence and competence in working with people with a variety of physical and cognitive impairments
  • Competence in basic IT skills with a willingness to learn how to use a variety of different software systems

Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK, access to a vehicle and be willing to travel.
