Advanced Practitioners work closely with their team, health and community colleagues to gain an in-depth understanding of resources that can facilitate and build independence, maintaining people’s ability to remain living in their own community networks and working with a strengths-based approach.Our ethos is to support Somerset residents to develop gloriously ordinary lives throughout our practice which means;

  • Utilising your extensive skills and experience to guide your team to support and empower the residents of Somerset to remain independent for longer in their communities.
  • You will be responsible for supervising registered Social Workers and Adult Social Care Practitioners.
  • Encouraging people to be independent, helping them grow their confidence and make the most of the support available to suit their individual needs.
  • Identifying what people and families are worried about or what they want to achieve, exploring what support they already have and what they need.
  • Developing great knowledge of your local area and working in partnership with local agencies.
  • Providing creative solutions.
  • Being a critical part of developing our new and exciting Adult Social Care service.

The teamThe Mental Health inpatient team is made up of 6 social workers and 4 adult social care practitioners and is overseen by an advance practitioner. We cover all 9 NHS Psychiatric hospitals across the whole of Somerset; supporting individuals prior to discharge from hospital.The service offers a hybrid way of working and due to covering the whole of Somerset, the team work hard to ensure time together is scheduled on a regular basis. This includes, shared lunches, cuppa and catch-up time, as well as meals/drinks in a social aspect.All members of the team are supported to progress within the team, should they wish. This includes training in specialist areas, such as BIA, Practice educator or progression to become an advance practitioner or Approved Mental Health Professional.Some feedback from the team around the best part of their job are that they get to work with people and watch their improvement, both physically and mentally. As well as being able to support people to realise their own potential and set goals for themselves.Working within the Mental Health inpatient team is a fast-paced environment, but you are guaranteed fun and laughter along the way due to the positive and fun nature of the team.We offer ongoing support, training and guidance to help you be the best you can be. But to be successful in this role you will:

  • Be a registered social worker with Social Work England with an enhanced continuous development record.
  • Have experience of working to an advanced social work level
  • Be autonomous in your decision making of adult safeguarding situations.
  • Have a strong sense of social justice and of working from a strengths based social work perspective.
  • Have a sound knowledge of the legal literacy within social care
  • Have access to/use of a vehicle is an essential requirement for this role

This is a Grade 9 post with a current salary of £40,476 to £44,711 per annum.
