Registered Nurse (RN) Case Manager – Home Care: Enjoy independence, autonomy, and flexibility in the field to visit patients one on one in their home environment.Hours: Full-time | Days | Monday – Friday | Holiday rotation (work approximately two holidays per year) | Weekend rotation may apply dependent upon staffing levels.Territory: This position will work in the field helping patients in their home setting. The office is in Roseville, MN and the general territory for this position would be the southwest section of the Twin Cities Metro area (i.e. cities would include those that are south of Highway 394 and west of 35W).Pay and Benefits:

• Annual Salary Range: $89,905 – $106,233

A few must-haves for this position:

• Graduate of an accredited school of nursing and has a current license as a Registered Nurse in the State of MN.
• Able to travel within a geographic area serviced by the office. Reliable transportation, valid drivers license, and proof of auto insurance is required.
• CPR Certification is required
• Two (2) years of current nursing experience.
• One (1) year case management experience preferred.
• Preferred: Community health and a wide range of Medical-Surgical experience.

Some functions of what you would do as a RN Case Manager:

• Perform assessments and procedures, which require knowledge, judgment, and nursing skill based upon principles of psychological, biological, physical and social sciences.
• Develop working relationships with discharge planners and social service workers.
• Assess and review the matching of employee skills to client needs.
• Visits in home clients to evaluate the quality of care being provided and review clinical notes.
• Participate in ongoing interdisciplinary assessment of the patient.
